Event Description
|
It was reported that the patient was hospitalized due to decompensated cardiac failure even though they had a cardiac resynchronization therapy pacemaker (crt-p) implanted.The device exhibited premature battery depletion associated with changes in the configuration of the device, specifically emergency mode, and exhibited a configuration failure.A device pacing algorithm that enhances cardiac resynchronization therapy was also noted to not be operating.The crt-p was explanted and replaced.No further patient complications have been reported as a result of this event.

Additional information received noted that the device was found to be in emergency mode when the patient was seen for extra-cardiac stimulation caused by the left ventricular (lv) lead.It was noted that the lv lead was not functioning in emergency mode and in subsequent telemetries showed intermittent capture failure while the device feature that monitors the pacing amplitude threshold and adjusts outputs was activated.Reprogramming was done and intermittent loss of capture was observed during subsequent programming.The lead remains in use.No further patient complications have been reported as a result of this event.

It was further reported that it was unknown how the crt-p was reprogrammed into emergency mode and the patient did not report being around a source of electromagnetic interference.The crt-p was reprogrammed out of emergency mode prior to explant.
