Event Description
Information was received from healthcare provider (hcp) via a manufacturer representative regarding a patient having spinal therapy for vertebral compression.It was reported that the fns tip was  very difficult to use, and to be sure it was correctly placed in the screw axe. extenders were bent during removal of the « en bloc » devices.Cement delivery guide was broken.Cement leaked and cement outside the vertebra was removed. longer procedure due to extra work (cement removal <(>&<)> screw replacement).Additional information received from manufacturer representative that the fns tip was not perfectly inserted in the screw.Fns mas screw, cap and bone filler device were not malfunctioned.There were roughly 45 minutes of extra work.Procedure involved was posterior fixation on a trauma case.The products came in contact with patient and removed «en bloc » with the other devices.There were no patient symptoms or complications reported as a result of this event.
