Event Description
It was reported the outside of the pressurewire x package was damaged as the seal on the box was broken and the sterility of the pouch was compromised.The end of the device was also noted to be bent.There was no clinically significant delay in the procedure and no patient involvement.No additional information was provided.
 
Manufacturer Narrative
Visual analysis was performed on the returned device.The reported broken seal on the box could not be confirmed as the packaging was not returned.The reported compromised pouch could not be confirmed as the pouch was inadvertently discarded by the inventory.Meanwhile, the bent could be confirmed.There was a kink on the distal tube and a bent on the sensor jacket.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no indication of a lot specific product quality issue.Based on the information provided and analysis of the returned device, a definitive cause for the packaging damage and kink/bent could not be determined; however, it was likely due to operational context.There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
