Event Description
It was reported that during bypass surgery the external pulse generator (epg) was set to ventricular demand mode (vvi) at 50 with sensitivity at 2 millivolts (mv) and an output of 20 milliamps (ma).The patient had an intrinsic heart rate of 80 beats per minute (bpm).Due to undersensing of the epg, pacing occurred r on t wave, resulting in ventricular tachycardia and ventricular fibrillation (vt/vf) with the need for defibrillation.The patient was successfully defibrillated without other complications.It was noted that the user did not evaluate sensing through the epg to confirm appropriate sensing.The epg was sent to the biomedical department for evaluation and it was noted that the biomedical engineer saw abnormal behavior when performing a safety check.No further patient complications have been reported as a result of this event.

Event Description
It was further reported that when the external pulse generator (epg) was evaluated the sensitivity function continued to gain without input.New cables were tried but the epg failed the sensitivity test repeatedly.
