Event Description

Information was received from a healthcare professional via a manufacturer representative regarding a patient undergoing t4-sai, l4-s right entry tlif for right root symptoms in spinal deformity and lumbar posterior scoliosis.It was reported that after implant placement, the set screw was threaded, but accidentally tried to thread the already threaded redu ction set screw on the right side of t9, and the tip of the reduction set screw breaker was buried in the set screw.It could not be removed.Although the tip was buried, it was not in a state that affected the construct.If the tip was removed forcibly, the possible situation that may occur was considered, so the removal was abandoned, and after that, the operation was completed as planned.Patient did not develop any symptoms or complications as a result of this event.No further complications were reported/anticipated.
