(b)(4).This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by ethicon inc, or its employees that the report constitutes an admission that the product, ethicon inc, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.Additional information was requested and the following was obtained: what is the status of the mesh? mesh is still implanted in the patient.It was after the procedure was completed that the surgeon examined the left over mesh to identify that it was fraying, falling apart at the edges.Small pieces were falling off as she stretched it out across a raytec.Patient current status/outcome? response from medical advisor: i have spoken with surgeon and the situation is currently resolved.The patient is not showing any signs of an adverse outcome.She will not be removing the product at this stage and will be monitoring the patient every 6 months for surveillance.Just checking if there any mesh parts are available for return? there is no mesh parts available for return.They were discarded.To date it has been reported that the device will not be returned.If the device or further details are received at a later date a supplemental medwatch will be sent.(b)(4).
 
Event Description
It was reported that a patient underwent an unknown procedure on (b)(6) 2022 and mesh was implanted.After the procedure was completed, the surgeon examined the left over mesh and identified that it was fraying and falling apart at the edges.Small pieces were falling off as she stretched it out across a raytec.The patient is not showing any signs of an adverse outcome and the product will not be removed.Additional information was requested.

(b)(4) this report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by ethicon inc, or its employees that the report constitutes an admission that the product, ethicon inc, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.Additional information: h6 photo analysis summary: neuchâtel team received for evaluation one photo of tvto device (product code 810081), batch 3941399.It can be observed in the photo that the device was manipulated.It can be also observed in the photo that the mesh is frayed with several particles around, and damaged.Additionally, organic matter can be seen around.The defect seen during the photo evaluation is aligned with the defect described in the event description, however, the defect identified is not linked to a manufacturing issue.
