Event Description
According to the reporter, during a laparoscopic cholecystectomy, while inserting trocar, blue thread-like parts were mixed when the new product was opened, and the obturator/trocar was ruptured.A new product was used to complete the case and resolve the issue.There was no patient injury.
 
Manufacturer Narrative
