According to the reporter, the capsule was deployed in the normal way but when the consultant came to withdraw the delivery device, it did not feel smooth and had to apply a little bit of pressure to remove it from the patient.The capsule came back still in site but then knocked off easily when put in the sharps bin.Another capsule was placed on the same procedure and it successfully attached.There was no patient and user harm.
