Event Description
A facility reported that they were unable to screw a levo adaptor into one of the starbursts because of what seems to be a burr within the threads of the mayfield skull clamp (a1059) starburst on one side.A swivel adaptor can be screwed into the skull clamp on both sides where the teeth engage but the levo adaptor cannot on one side.The teeth would not engage.The scheduled surgery was a posterior cervical fusion which is what the surgeon proceeded with after successfully pinning the patient.A 15 minutes delay was noted as they had to switch out the skull clamp.

Attempts are being made to obtain additional information.Upon completion of the investigation, a follow-up report will be submitted.

Mayfield modified skull clamp (a1059) was returned for evaluation: device history record (dhr): the dhr was reviewed and shows no abnormalities related to the reported failure.Failure analysis: the unit was returned in good working order, no maintenance is required at this time, unit passes inspection per quality inspection checklist 1101.The customer was using a levo adaptor and not a mayfield adaptor.Per the mayfield a1059 ifu ¿we caution that we have only validated mayfield products used in conjunction with the mayfield product line, and as a result, cannot advise you whether suppliers¿ products would work properly with the mayfield line of products.¿ root cause: complaint not confirmed via inspection of the unit.The returned unit was in good working order and no maintenance was required.No further investigation required based on the acceptability of risk and no adverse trends identified.This will be monitored and trended going forward.At present, we consider this complaint to be closed.
