Updated fields: d4, d9, g3, g6, h2, h3, h4, h6, h10 the mayfield skull clamp (a1059) was returned for evaluation: failure analysis - the investigation of the returned unit showed that the reported complaint was confirmed from the evaluation.The unit had a loose swivel lock assembly, and some worn off small parts were preventively replaced to readjust the swivel lock assembly.Afterwards function of the skull clamp was performed.Root cause - the complaint is confirmed via inspection of the unit.The swivel lock assembly was loose and required replacement of worn components from routine use and wear.Additionally, improper or suboptimal placement of the skull clamp on the patient can contribute to unintended movement/slippage of the patient.No further investigation is required based on the acceptability of risk and no adverse trends were identified.This will be monitored and trended going forward.At present, we consider this complaint to be closed.
