Event Description
It was reported that a patient underwent an idvt ¿ left ablation procedure with a thermocool® smart touch® sf bi-directional navigation catheter.The patient suffered cardiac tamponade requiring a pericardiocentesis.The patient suffered a pericardial effusion.Towards the end of the case, the physician was using intracardiac echocardiography (ice) to check his catheter position and noticed the effusion.A transthoracic echo was performed to confirm the pericardial effusion.A pericardiocentesis was performed and about 15 cc of fluid was removed from the pericardial space.Patient is stable currently and was stable throughout the entire procedure.No ablation was performed as there was no area to ablate.The physician thinks that the effusion happened from the transseptal using the agilis sheath.They are unsure if the catheter has been preserved for return and noted that the packaging has been discarded so the lot is unknown.Additional information was received.The adverse event was discovered during use of biosense webster products.Physician¿s opinion on the cause of this adverse event was transseptal procedure.Patient was stable while in the lab.Unknown if patient required extended hospitalization because of the adverse event.Relevant tests/laboratory data-echo.Other relevant history-unknown.Generator information-unknown.Transseptal puncture was performed with a baylis needle.No evidence of steam pop.The event occurred during the transseptal phase.No radio frequency was given.The device is not available for return.
