It was reported that during a hip procedure, the inserter broken.There was no reported harm or injury to the patient.Attempts have been made and additional information on the reported event is unavailable at this time.

(b)(4).Concomitant medical products: 00875705800 ¿ continuum shell ¿ 65253776.Visual examination of the provided pictures identified the tip of the inserter fractured and the broken piece remained in the shell.Part and lot identification are necessary for review of device history records, neither were provided.A definitive root cause cannot be determined.Initial report was reported under the wrong mfr number, see 0001825034-2022-02726.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.

This follow-up report is being submitted to relay additional information.The following sections were updated/corrected updated: g3; h2; h3; h4; h6 visual examination of the returned product identified nicks, gouges, scratches, and indentations from previous uses to all areas of the handle, body, and strike plate.Hex bolt is confirmed to be fractured.Review of the device history records identified no deviations or anomalies during manufacturing.Root cause unchanged.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
