|
Medtronic received information that a react-71 catheter distal tip was stripped or ruptured.Mechanical thrombectomy (mt) with continues aspiration on neuron max, react and preset simultaneous retrieved, aspiration catheter was sucked in the y connector.The physician, assumed that the distal tip of the react striped of and did a anterior occlusion.The patient had a anterior infarct.The anterior has a contralateral supply.No injury at the end.Ther catheter was flushed as per the instructions for use (ifu).The devices were prepared as per ifu.The patient was being treated for a mt.Vessel tortusoity was moderate.The access vessel was the middle cerebral artery (mca) with a diameter of 3mm.

Additional information received reported that the catheter sucked in the y connector because of the suction on the neuron max.There was no patient/therapy issue.The cause of the event was not determined.
