The device was not returned to olympus for evaluation.The customer resolved the issue onsite by draining the air and replacing the water filter.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus that the water flow in the reprocessing basin decreased causing an increase in the reprocessing cycle time.It was the first cycle of the day.There were no reports of patient harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the approved final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the water flow issue could not be determined.It is possible that the water flow decreased because the user did not read the instruction manual carefully and lacked knowledge of the replacement method of the water filter.The instruction manual identifies the following verbiage, which may have prevented the phenomenon: ¿8.4 replacing the water filter (maj-824 or maj-2318): caution: if air gets in the water filter housing, it may extend the process time.In case of an irregularity such as extension of the process time or lack of water supply, drain the air as described in ¿ draining air in the water filter housing¿ on page 402.¿ olympus will continue to monitor field performance for this device.
