Event Description
During a ventricular tachycardia ablation procedure, the catheter became entangled, and damage was noted to the device upon removal from the patient.The catheter was advanced through a non-abbott (biosense webster vizigo) steerable sheath into the right atrium to collect geometry.The catheter was difficult to insert, and an attempt to pull the catheter back into the sheath was unsuccessful as resistance was noted.Fluoroscopy revealed that the catheter was inhibited by the non-abbott device (biosense webster ez steer cs).The two catheters had become entangle together during catheter manipulation.Once the catheter was freed from the non-abbott device, the catheter was removed, and the tip was noted to be damaged.Another device of the same type was used to complete the procedure no consequences to the patient.
 
Manufacturer Narrative
One bi-directional, curve d-f, sensor enabled, advisor hd grid mapping catheter was received for evaluation.The distal coupler was displaced and the pellethane tubing was torn.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.The cause of the insertion difficulty, torn pellethane tubing, and displaced distal coupler remains unknown.
