Information received by medtronic indicated that the customer experienced a hyperglycemia, hypoglycemia ,under delivery , over delivery alert , visited emergency room and hospitalized for treatment.Customer blood glucose level was over 500 and 50 mg/dl.Customer also noticed crack on retainer ring damage.It was unknown about used insulin pump system within 48 hours of reported event.Troubleshooting was not performed for blood glucose readings and it was unknown about used of the minimed 670g/770g/780g system with the auto mode/smartguard feature actively at time of blood glucose event.Customer was advised the insulin, reservoir, and infusion set will be replaced.The products will not be returned for analysis.Frn-mmt-332-rsvr, unomed inf set.
