It was reported to the manufacturer, by the end user, per the end user, that the patient had fallen out of bed.I advised the pt has a dflal and the bolsters could be inflated.She said she didn't know if they were and put me on hold to go check.Another lady came on the line and said the bolsters were not inflated.I thought i was speaking with the same lady.She was very rushed and spelled out her name.I explained that i need to file a report on the fall so she put me on hold to go get the patients nurse.Rn came on and she confirmed that patient fell but there were no witnesses and no medical attention was needed.I advised i had created a service to have the current lal mattress swapped out and the fst will inflate the bolsters on the new dflal.Service order (b)(4).Complaint #(b)(4) and ra #(b)(4) was entered into our system to have the mattress and control unit returned for investigation.As of this writing, the products have not been returned.
