Philips has investigated this case.According to the information collected, the customer inquired about the wireless footswitch replacement status for a previously reported defective wireless footswitch, which was addressed in complaint tw 1996801.Philips informed the customer regarding the availability of the replacement wireless footswitch and the service case was closed.The customer continued to use a wired footswitch until the replacement footswitch was later provided (tw 3640240).The system has been returned to use in good working order.At the time this complaint was received, philips did not have enough information to exclude the potential for death or serious injury on recurrence, and as such the complaint was reported.Since that time, new information has been received which has led to the determination that this is scenario is not likely to cause or contribute to death or serious injury if it were to recur.There was no reported product malfunction in this case.The customer had requested information regarding the availability of a replacement wireless footswitch.Philips has re-evaluated this case as not reportable.Codes were updated based on investigation results.Device problem code was corrected.
