Event Description
It was reported that during the procedure, the surgeon attempted to use the reamer guide inserter to insert the ream guide and it was very difficult to pull the lever.After the case, we attempted to lubricate the instrument but it is still difficult to use.It is also making a cracking noise and seems bent.No adverse events occurred and there was no delay in surgical time.

Product complaint # (b)(4).Investigation summary: the device associated with this report was returned to depuy synthes for evaluation.Visual examination and handling of the device found the lever really hard to pull that it can be considered functionally jammed, a bent condition cannot be seen.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.Corrected: h3.
