The subject device was returned to olympus for evaluation.During inspection and testing, the instrument channel had a leak.Distal end of device had loose/deep cut on probe pink rubber.The biopsy channel was leaking.Bending section cover glue had a crack.Light guide lens had uneven seal.Two consecutive elements 10 and 11 were broken.Bending section cover had water invasion.There was a missing name plate on control body cover.The control knob in up/down direction play was loose.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
A customer reported to olympus, the evis exera ii ultrasound gastrovideoscope plate fell off and black material was coming out of the scope.There was no report of patient harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported "plate falls and foreign matter comes out¿ could not be determined, as no additional information was available.Olympus will continue to monitor field performance for this device.
