A field service engineer (fse) was at the customer¿s site to address the reported event.Fse confirmed the complaint by reviewing the result printouts and did not attempt to reproduce the error.While troubleshooting, fse found clotting in the sampling nozzle assembly causing the analyzer to aspirate an incorrect volume.Fse resolved the complaint by cleaning the sampling nozzle assembly.Fse repaired and validated the analyzer by successfully performing daily check and quality control run without error and within acceptable range.No further action required by field service.The aia-2000 analyzer is functioning as expected.A 13-month complaint history review and service history review for similar complaints was performed for the serial number (b)(4) from 02jan2022 through aware date 02feb2023.There were no similar complaints identified during the search period.The most probable cause of the reported event was due to clotting of the sample nozzle assembly.
 
Event Description
A customer reported ¿visually seeing a clot in the sample nozzle tip¿ upon inspection of the sample nozzle on the aia-2000 analyzer.Prior to the call, the customer had flushed and cleaned the sample nozzle of serum residue.All quality control (qc) is running out of range low, the customer opened a new set of quality controls and reran, the values are still low.The analyzer is down.A field service engineer (fse) was dispatched to address the reported event which resulted in a delayed reporting of patient samples for alpha-fetoprotein (afp).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.
