This will be filed to report a cracked clip introducer.It was reported that during device prep of a mitraclip procedure, while the clip delivery system (cds) was being inserted into the clip introducer, the clip introducer was entered between the clip and the shaft.Subsequently the clip introducer cracked.The cds was exchanged and the procedure was completed without further complication.There was no clinically significant delay in the procedure and no patient involvement.No additional information was provided.

All available information was investigated, and the reported cracked clip introducer was confirmed.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents reported from this lot.Based on available information, the reported cracked clip introducer was due to procedural circumstances.There is no indication of a product quality issue with respect to manufacture, design or labeling.H6: code 1069 removed.
