|
It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a carto vizigo¿ 8.5f bi-directional guiding sheath ¿ medium and the sterilization was compromised.When setting up for an ablation case, upon opening the packaging of the carto vizigo¿ 8.5f bi-directional guiding sheath ¿ medium, it was noticed that the "top of the packaging seemed to be open" and they believe that the "sterility was compromised".To troubleshoot the sheath was replaced and the procedure continued.There was no patient consequence reported.Additional information was received.There was no visible damage to the outer box/packaging.The device was secured in the packaging tray correctly.There was no visible damage to the device.The pouch seal that was in question was opened further by staff before making the biosense webster representative aware of the issue.The device was not used on the patient.The event was assessed as mdr reportable for sterilization compromised.

The bwi product analysis lab received the device for evaluation on (b)(6) 2023.The analysis has begun but is not completed at this time.When the investigational analysis has been completed, a supplemental 3500a report will be submitted.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by biosense webster, inc., or its employees that the report constitutes an admission that the product, biosense webster, inc., or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.Manufacturer's reference number: (b)(4).

It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a carto vizigo¿ 8.5f bi-directional guiding sheath ¿ medium and the sterilization was compromised.The device evaluation was completed on 04-apr-2023.The device was returned to biosense webster (bwi) for evaluation.Bwi conducted a visual inspection evaluation of the returned device.Visual analysis revealed no damages or anomalies on the device.It was stated that the issue reported was related to the packaging of the device.Since the original package was not returned, no analysis could be performed.A device history record evaluation was performed and no internal actions related to the complaint were found during the review.As part of bwi¿s quality process, all devices are manufactured, inspected, and released to approved specifications.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
