Event Description
A customer reported to olympus, the evis exera ii ultrasound gastrovideoscope had a black leakage.The event occurred during reprocessing.There was no report of patient involvement.
 
Manufacturer Narrative
The subject device was returned to olympus for evaluation.During inspection and testing, the biopsy channel had a leak.The plastic distal end cover had minor scratches.The ke45 was missing at forceps cover.The bending section cover glue was cracked.The switch button 1 was cut.The control body, scope connector and ultrasound connector had fluid invasion and corrosion.The lock engagement lever had deep scratches.The ultrasound electrical insulation failed.The play of the up/down and right/left angulation control knobs were loose.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the reprocessing could not be completed and foreign material remained because of a leak failure occurred at the biopsy channel.The foreign material was unable to be identified.The event can be prevented by following the instructions for use: "chapter 6 compatible reprocessing methods and chemical agents chapter 7 cleaning, disinfection, and sterilization procedures" olympus will continue to monitor field performance for this device.
