Information was received from a healthcare provider regarding a patient who was implanted with an implantable neurostimulator (ins) for unknown indications for use.It was reported that the set screw was loose and impedance was greater than 20,000.The device was explanted on (b)(6) 2023.

|
Additional information was received from a manufacturer representative (rep).The rep reported that it is unknown what the most likely cause of the loose set screw was.The device was replaced.The patient's symptoms have resolved.The rep is uncertain if the cause of the high impedance was determined - the hcp believes the issue is with lead port 2 and the set screw not being seated.In response to the most likely cause of this issue they reported "unsure, thinking it may have been a cross threaded set screw on lead port 2." this issue was resolved after the device was replaced.
