The device was returned to olympus for evaluation and the customer's allegation was able to be confirmed.During the device evaluation, it was observed that the probe body was damaged in the middle of the probe.Upon further inspection, that it was observed that the unit failed to display the ultrasound image, and that there was a bubble near the probe tip.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The customer reported, to olympus the ultrasonic probe unit tip was peeling.The reported issue occurred at pre-wash during reprocessing of the scope.There was no patient/user harm or injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the damaged sheath occurred due to accidental failure or a failure due to handling.However, a definitive root cause could not be determined.The following information is stated in the instructions for use (ifu): ¿[important information ¿ please read before use].Do not hit, stretch, twist, drop, or bend excessively the distal end, insertion section, or connector section of the ultrasonic probe.Otherwise, the equipment may be damaged, causing an injury in the body cavity, burns, bleeding, perforation, or detachment of parts.¿ olympus will continue to monitor field performance for this device.
