According to the reporter, in a laparoscopic procedure, there were issues with the loading or firing of the clips.The clip applier jammed with the jaws open.The clip applier could not come out of the trocar, so the surgeon had to remove the trocar as well to beable to get the clip applier.Another clip applier with the same lot number was opened but the light would not come on, so the device could not be used.Another device was used to complete the case.There was no patient injury.
