Event Description
The customer reported to olympus, the evis lucera elite colonovideoscope exhibited angulation malfunction.There was no movement when the angle knob was turned.The issue occurred during an unknown procedure.The procedure was completed with a similar device.The subject device was returned and an evaluation at the repair center revealed white contamination, possibly simethicone type product, inside the auxiliary channel.This report is being submitted to capture the reportable malfunction found during evaluation.There were no reports of patient harm associated with the event.
 
Manufacturer Narrative
An evaluation of the returned device confirmed the customer allegation, ¿no movement when the angle knob was turned¿.The angulation in the upward direction was inoperative.There were few additional findings: the distal end adhesive was lifting, the insertion tube had minor delamination.The investigation is ongoing.A supplemental will be submitted on completion of investigation or if any additional information is received.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation and correction to b3.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the foreign material was insufficient cleaning.Identification of the material was a simethicone type product.Olympus will continue to monitor field performance for this device.
