Event Description
An attempt was made to use one 5f dxterity diagnostic catheter.It was reported that the dxterity catheter became kinked in the left subclavian artery while in the patient.The device could not be removed.Assistance was needed by a vascular surgeon to remove the catheter.The catheter was removed percutaneously.The vascular surgeon performed an angiogram of the radial and subclavian artery and no dissection was seen.A coronary angiogram revealed triple vessel disease and the patient was referred to another facility.Patient complained of shortness of breath approximately an hour after the procedure.It was reported that the patient died soon after.
