Event Description
Patient was receiving antiemetic medication through port when he noticed that his shirt was wet.I paused the infusion, and investigated.The leak seemed to be coming from where the syringe tubing was connected to the hazardous trifuse extension set.When the syringe tubing was disconnected, the inner part of the clave it was connected to was still engaged.Switched out trifuse and continued infusion with no leaking.
