Complainant alleged that while attempting to cardiovert a 65-year-old male patient, an arc was seen from the electrode pads.Complainant indicated that there was no adverse effect to the patient due to the reported malfunction.

Zoll medical corporation has not received the product for evaluation and this complaint is still under investigation.

The pads were returned to zoll medical corporation for evaluation without the original packaging opened.The customer's report was not replicated or confirmed.The pads were stuck to a napkin and were unable to be separated for any testing.A visual inspection of the returned electrode pads found hair and discoloration around the tins of the pads.The pads were scrapped after testing.The instructions for use (ifu) for the pro-padz electrodes provides instructions on proper skin preparation and electrode application.The ifu also warns: excessive hair can inhibit good coupling (contact), which can lead to the possibility of arcing and skin burns; poor adherence and/or air under the electrodes can lead to the possibility of arcing and skin burns.Analysis of reports of this type has not identified an increase in trend.
