It was reported that the patient presented during implant procedure.During procedure, perforation was noted before the left ventricular lead was implanted.The reported lead was not implanted.The patient was in stable condition.

Additional information received noted that the lead perforated during implant procedure while the guidewire was inserted, causing insulation damage.The patient did not experience perforation was stable after the procedure.No further interventions were performed.

Correction: b1, b2, b5, h1 - patient perforation was erroneous reported in the lead was perforated and there was no perforation noted on the patient.The reported event of lead insulation damage was confirmed.As received, a complete lead was returned in one piece for analysis.Visual inspection found a small hole on the lead body insulation consistent with damage caused by puncture of the wire.No other anomalies were identified.
