Event Description
Medtronic received information that during use of a autolog iq instrument, it was reported that a little more than 2000 ml of blood was collected in the cardiotomy reservoir, together with 700ml from the ekc system (hematocrit 23%).There was a failure in the collection of single 135ml bowl, and all the blood ended up in the waste bag.The blood collected for the final content was washed and returned to the patient.The instrument was used to complete the procedure.There was no adverse patient effect associated with this event.The customer stated that the issue was related to a sensor error, or a problem with the imbalance of the centrifuge.The centrifugal bowl was well placed and connected.Medtronic received additional information that the blood (600ml) sent to the waste bag was discarded.The patient did not require a transfusion of non-autologous blood because of the blood sent to the waste bag.It was reported that the sensor did not recognize a sufficient amount of hematocrit (htc) for washing.
