The mayfield skull clamp (a1059) was returned for evaluation: failure analysis : the reported complaint was confirmed from the evaluation.The ratchet extension had a protruding threaded insert.After the internal thread has been visually checked and no defects can be seen on the aluminum frame, the thread insert was replaced, the 80 lb.Jackscrew was checked and the external thread was recut as a precaution.The skull clamp was then subjected to a successful functional check.Root cause : complaint is confirmed via inspection of the unit.The ratchet extension had a protruding thread insert (heli-coil) that required replacement.Additionally, improper or suboptimal positioning of the skull clamp can contribute to patient slippage or movement.No further investigation is required based on the acceptability of risk and no adverse trends were identified.This will be monitored and trended going forward.At present, we consider this complaint to be closed.
