A.1.-a.5.There was no patient involvement.H10: livanova deutschland manufactures the heater-cooler system 3t.The incident occurred in (b)(6).Through follow-up communication, livanova learned that cardioplegia side was working properly, while pumps on patient side seemed to not work at all.No alarm message was seen by the customer.If any additional information pertinent to the reported event is received, it will be provided in a supplemental report.
 
Event Description
Livanova deutschland received a report that a heater-cooler system 3t was not working at all on patient side.The issue occurred during maintenance activity.There was no patient involvement.
 
Manufacturer Narrative
No similar events were reported since unit installation in 2008.Based on available information, considering the age of the unit, it cannot be excluded that a damaged patient pump (pump 1, pump 2 or both) caused the reported failure, due to wear.The wearing of electro-mechanical device can be originated by the specific use condition at customer site and may have contributed to the event, however there is no concerning trend for this kind of failure.The risk is in the acceptable region.No specific action was currently deemed necessary.Livanova will keep monitoring the market.
