Livanova deutschland received a report that a heater-cooler system 3t displayed an error message associated to patient pump 2 blocked (too slow) during procedure.There was no patient injury.

Manufacturer Narrative
A livanova field service engineer was dispatched to the customer site to investigate the issue.Patient pump 2 replaced.Subsequent functional verification testing was completed without further issues and the unit was returned to service.If any additional information pertinent to the reported event is received, it will be provided in a supplemental report.

The unit is installed since 2017 and a review of the complaint database did not reveal further similar reported issue related to this heater cooler.Based on all the above facts, most likely root cause of the reported event is reasonably assigned to environmental conditions in which the pump worked.Indeed, water infiltration or water formation due to condensation or high temperatures and high level of humidity in the stationary phase may have caused its failure.The wearing of electro-mechanical device can be originated by the specific use condition at customer site and may have contributed to the event, however there is no concerning trend for this kind of failure.The risk is in the acceptable region.No specific action was currently deemed necessary.Livanova will keep monitoring the market.
