Event Description
|
It was reported that the cardiac resynchronization therapy defibrillator (crt-d) was unable to communicate with a programmer.The programmer head was disconnected from the programmer and placed over the crt-d and no tone was produced.When the programmer head was connected to the programmer no lights were produced to indicate it was able to detect the crt-d.A mobile programmer was attempted but was unable to communicate with the crt-d.According to external electrocardiogram (ecg) the crt-d was pacing at the previously programmed rate.It was noted that the crt-d has not had a remote transmission for two years.The crt-d remains in use.No patient complications have been reported as a result of this event.

It was further reported that the crt-d had twelve months of longevity remaining.Subsequently, the crt-d was able to be interrogated.The crt-d was explanted and replaced.
