Event Description
It was reported that, after a thr had been performed on an unknown date, the patient experienced a dislocation.A revision surgery was performed on (b)(6) 2023 to address this adverse event.One (1) unknown tandem unipolar fem head was explanted and a s+n's head (ox head was implanted.The cup (tandem) and the stem (anthology) remained.Patient's current health status is unknown.

H3, h6: the device was not returned for evaluation; therefore, a device analysis could not be performed.The clinical/medical investigation concluded that, this case reports a revision hip replacement performed due to dislocation in which the tandem unipolar femoral head was exchanged for a oxinium head.As of the date of this medical investigation, the requested clinical documentation has not been provided for evaluation.It has been communicated via e-mail that no further information is available.Therefore, there were no clinical factors found which would have contributed to the reported dislocation and subsequent revision.Per case details, the surgeon doesn't think it is product failure.The patient's current condition is unknown and the patient impact beyond the revision surgery could not be determined.No further clinical assessment can be rendered at this time.Should additional clinical documentation become available in the future, the clinical/medical task may be re-evaluated.Device specific identifiers were not provided.Therefore, an evaluation of the manufacturing records, complaint history review, instructions for use, risk management file and prior actions review could not be performed.At this time, we have no reason to suspect that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include traumatic injury, patient anatomy or abnormal loading of limb.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened.No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary.We consider this investigation closed.
