Event Description
On (b)(6) 2023 accelus was informed of a complaint on a linesider mis screw.It was reported that the tabs broke at the incorrect location during the procedure leaving a portion of the extended tab in the patient.This went unnoticed until review of post-op imaging.No adverse patient symptoms were reported as a result of the retained tab, however, the surgeon intends to perform a revision surgery to remove the extended tab.The date of the revision surgery is unknown at this time.
