Event Description
The customer reported to olympus that during therapeutic endoscopic submucosal excavation procedure, this single use ligating device was used for ligating the wound.The loop was detached from the front end of the handle and domestic accessories were used to operate the loop and suture the wound.The procedure was delayed for 30 minutes while the patient was under general anesthesia.There were no reports of further patient or user harm associated with this event.

This device has not been returned for evaluation as it was discarded.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or when additional information becomes available.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the reported event occurred because the slider was accidentally pushed.This likely caused the loop to detach from the device.However, the root cause could not be determined.The event can be detected/prevented by following the instructions for use (ifu) which state: ¿do not push the slider when inserting the instrument.The loop may come off the hook and get stuck inside the instrument channel of the endoscope.¿ although the subject device was manufactured in august 2022, the specific date is unknown.Olympus will continue to monitor field performance for this device.
