Event Description
This is filed to report a clip opening while locked (cowl) and clip jumping, requiring intervention.It was reported that a mitraclip procedure was performed to treat degenerative mitral regurgitation (mr) grade 4.After lock line removal during the deployment process, the clip jumped open from fully closed to approximately 40 degrees.Troubleshooting was performed but was unsuccessful.Subsequently, the physician deployed the clip.It was noted that the arm positioner rotated freely, and the clip no longer responded to communication; a cable break was suspected.A second clip was then implanted lateral to the cowl clip for stabilization.The procedure was concluded with a resulting mr of grade <1.There was no clinically significant delay in the procedure and no adverse patient sequelae.No additional information was provided.

The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified did not indicate a lot-specific quality issue.Based on available information and without the device to analyze, the cause of the reported clip opening while locked & clip arms jumping during the procedure could not be determined.The cause of the reported unstable (arm positioner) & break (cable break) cannot be determined.The reported unexpected medical intervention was a result of case-specific circumstances.There is no indication of a product quality issue with respect to manufacture, design, or labeling.
