It has been reported to philips that there was startup issue due to generator being busy.The voltage generator for the x-ray tube went into error and angiography was stopped and was disconnected from the hospital's electrical voltage.After refueling the angiograph and turning it on, its functionality was normal, without any other error messages.The system was not in clinical use at the time of the event.No harm has been reported to philips.Philips has started an investigation of this complaint.

Philips has investigated this complaint.The philips field service engineer (fse) inspected the system onsite and confirmed that there were ¿generator error.¿.Review of the log file confirmed voltage variation in one of the angiograph power supply phases.It was confirmed that the issue occurred due to defect in the costumer¿s electrical circuit.Fse refueled and turned-on system, the system was returned to use in good working order.At the time this complaint was received, philips did not have enough information to exclude the potential for death or serious injury on recurrence, and as such the complaint was reported.Since that time, new information has been received which has led to the determination that this is scenario is not likely to cause or contribute to death or serious injury if it were to recur.The procedure was completed using the same system as there was issue with customer¿s electrical circuit.There was no impact to the patient.Based on the new information, this complaint is evaluated as not reportable.The codes were updated based on the investigation outcome.Device problem code was corrected.
