Event Description

It was reported that during a cryo ablation procedure, there was a console failure.The case was aborted while the patient was under general anesthesia. no further patient complications have been reported as a result of this event.

(b)(6) 2023: it was later reported that a system notice was received indicating that the system did not recognize the catheter.Troub leshooting was performed and the balloon catheter was replaced without resolution.The auto connection box was then replaced which resolved the issue.It was reported that an internal pin of the auto connection box was cut and bent, preventing the catheter from being electronically recognized.
