Event Description
|
Physician attempted to use a protégé rx self expanding stent with a 5mm spider fx embolic protection device during patient treatment.The ifu (instruction for use) was followed during preparation, procedure, post-procedure.There was no damage noted to packaging (i.E.Shelf carton, hoop/tray).There were no issues noted when removing the device from the hoop/tray.The vessel was not pre-dilated but was post-dilated.The device did not pass through a previously-deployed stent.No resistance was encountered when advancing the device.After the spider was in place, when pushed the carotid artery stent, it was found that the guide wire at the tip of the spider was broken.At this time, the stent had just been half deployed, and the operator terminated the surgery for the safety of the patient.After the spider and stent were taken out of the body together, it was found that the guide wire of the umbrella had slipped and was not completely broken.However, it could not continue to be used after a doctor's evaluation.No intervention required for the removal of the devices and were safely removed from the patient.No vascular damage visible on x-ray.A replacement spider and stent were used to complete procedure successfully.No patient injury reported.
