Event Description
A physician was attempting to implant a protege rx for the treatment of a plaque lesion in the common carotid artery.There was moderate tortuosity and calcification.The device was prepped as per the ifu with no issues identified.It was reported that resistance was encountered during delivery to the lesion and force was applied.The device passed through a previously deployed stent.The device could not deploy.No intervention was required for the removal of the device, stent struts were exposed/visible on removal and the device was safely removed from the patient.Another device was used to complete the case.No patient injury was reported.
