Event Description
Information was received from a healthcare provider (hcp) and a manufacturer representative (rep) regarding a lead.It was reported that the rep called to request a return product mailer for lead.The rep said the lead sheathing stripped away when the doctor inserted it through the introducer needle.The rep said they already filed the event with their department's medical team and will complete manufacturer representative's form also.Technical services (ts) ordered the return mailer for the hcp.
