|
Information was received from a manufacturer representative regarding a patient who was implanted with an implantable neurostimulator (ins) for unknown indications for use.It was reported that patient came in for revision replacement of the lead for mri compatibility as patient required mri, therefore mri compatible lead was needed.The neurostimulator was going to be reused as it still had battery life and there was no issues with it.When attempting to connect the neurostimulator with the new mri compatible lead, the set screw did not lock the lead into place, even though it sounded and looked like it was locking properly but the lead kept sliding out of the neurostimulator.They attempted to lock the set screw in place multiple times with no results.Even when tightening the set screw without the lead, the surgeon was able to still slide the lead into the neurostimulator, indicating that it wasn't locking properly.The neurostimulator was removed from the patient and replaced with the interstim x.Issue resolved. field contact in possession of removed product, will be returned appropriately.Patient was implanted with an interstim x neurostimulator and therapy continues as before.
