Event Description
It was reported that the plugged tip of the unused extension eroded through the skin at the lead to extension incision site.Surgical intervention occurred and the plugged extension was exposed.A trough was created in the skull and both extensions were secured within the trough.Additional information received from the manufacturer¿s representative (rep), which was confirmed with the healthcare provider (hcp), reported the erosion was due to tissue pressure against the tip of the plug in the extension.The issue was resolved through a revision at the lead to extension connection site.
