Event Description
The customer reported to olympus, the evis lucera gastrointestinal videoscope image suddenly flickered, and a mosaic appeared.The issue was found in the middle of a diagnostic gastroscopy, the procedure was completed with a similar device, and there was a 3 minute delay (the patient was under general anesthesia).There were no reports of patient harm.
 
Manufacturer Narrative
The device will not be returned to olympus for evaluation.The investigation is ongoing, a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and because the device was not received for evaluation, the definitive root cause of the image issue could not be determined.The instruction manual identifies verbiage which may have prevented the phenomenon in the "inspection of the endoscopic image" instruction manual.Olympus will continue to monitor field performance for this device.
