The device was returned to olympus for inspection, and the customer's reported issue of ¿37 inches of sheath stripped the full length of the probe¿, the insertion tube is separated from the shaft was confirmed.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

The customer reported to olympus that the ultrasonic probe had ¿37 inches of sheath stripped the full length of the probe¿, the insertion tube is separated from the shaft during maintenance.There was no report of patient harm or user injury associated with this event.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Please see updates to h6 and h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the final root cause of this event (insertion tube separated from the shaft) was unable to be identified.Olympus will continue to monitor field performance for this device.
